I hereby request and authorize the opportunity to observe a clinical skincare treatment performed by licensed esthetician, Becky Eder. I understand that this is a strictly educational observation and does not constitute professional training, certification, or the right to perform these treatments myself.
Confidentiality & Privacy (Non-Disclosure)
I understand that during the course of this observation, I may be exposed to the private health information, medical history, and physical appearance of a client. I agree to maintain the absolute confidentiality of the client being treated. I will not record, photograph, or share any identifying details of the client or the treatment process without explicit, written consent.
Observation Protocol
I have been informed of the nature and purpose of this observation. I agree to remain in the designated observation area and will not interfere with the treatment, touch the client, or handle any clinical tools or products unless specifically directed by the esthetician. I understand that the esthetician reserves the right to terminate the observation at any time if she deems my presence is distracting or detrimental to the client’s experience or safety.
Assumption of Risk & Liability
I recognize that a clinical environment contains specialized equipment. I assume all risks associated with my presence in the treatment room. I do not hold Becky Eder or her practice responsible for any accidental injury or incident that may occur during my observation period.
Professionalism & Conduct
I have, to the best of my knowledge, disclosed any illness or condition that might pose a risk to the clinical environment. I have read and fully understand this agreement. All of my questions regarding the observation protocol have been answered to my satisfaction, and I consent to the terms of this agreement.
